The May 5 workshop produced something rare: 74 ideas, 7 priorities, one clear lighthouse, and a leadership team aligned on sequencing. This proposal doesn't restart that work — it operationalizes it. Most named initiatives below sit in Bucket 01 of the opportunity report; the Patient Concierge is a strategic late-add that bridges patient experience and operations in a single move. The wildcard protects the team's ability to pivot on something that wasn't on the radar yet. The enablement layer makes sure the AI tools P1 already pays for actually get used.
Our approach: de-risked by design. A 90-day proof-of-value milestone gives P1 a clean decision point at the engagement's halfway mark — if the work isn't showing returns, the engagement ends.
What we'll actually build
over six months.
Five targeted initiatives, one wildcard, and an always-on enablement layer. Most named workstreams map to a Bucket 01 priority from the opportunity report; the Patient Concierge is a late-add bet on patient-facing experience that touches scheduling, collections, and support in one move. Each initiative is scoped to ship something working in the first 90 days. The wildcard is intentional — it protects P1's ability to redirect when something surfaces in-quarter that matters more than what we planned.
Schedule Fulfillment
The #1 voted use case from the workshop. Build a smart-scheduling agent that routes unscheduled patients into open chair time across pilot practices — quantifies revenue leakage, surfaces utilization gaps to RDOs, and protects against double-booking. High impact, very feasible: Ascend API access is already approved and the source data exists.
AI Recruiting Companion
An AI companion supporting P1's HR and recruiting teams across both general hygiene/dental sourcing and specialty endodontist relocation. Cross-references LinkedIn, alumni networks, geographic ties, and personal context to score candidates and draft personalized outreach. Maps directly to Graham's two "genie wishes" from the April 9 call.
Personalized Practice Business Reviews
Auto-generated, practice-specific monthly business reviews — replacing the static-for-everyone reporting Wes's team is drowning in. Pulls from the existing Power BI foundation, but adds a tailored AI layer that learns each practice's patterns, surfaces what's changed, and proposes specific operational moves. Less dashboard, more "what should I do this month?"
Patient Concierge
An AI-powered patient concierge that serves as a 24/7 front door for every P1 practice. Handles scheduling questions, sends payment reminders, walks patients through balance inquiries and payment plans, and answers practice-specific FAQs. Each practice gets its own voice, hours, and policies — but the underlying intelligence is shared across the network, so a pattern learned at one location improves the experience everywhere.
The Wildcard slot
A reserved workstream with no pre-defined scope. Held intentionally for the priority that doesn't exist yet — the acquisition that lands mid-engagement, the regulatory shift, the payer disruption, the Prairie-driven directive, or the use case from the workshop's Bucket 02 that suddenly matters more than the plan. Activated by joint decision between Graham and the engagement team.
Fractional AI Enablement Layer
A continuous, lightweight layer of AI support that runs across the entire six-month engagement. Helps P1 get more value from the AI tools and licenses you're already paying for. The intent: an AI partner the team can actually reach. If Wes wants to learn how to automate a report in Claude, he reaches out. If Maureen wants a workflow review, she books office hours. If the AI council wants someone in the room — we're there.

with a 90-day decision point.
We don't ask P1 to commit on faith. The first 90 days are structured to deliver working proof of value across the named workstreams. At day 90, P1 has a clean opt-out: continue if the evidence is there, walk away if it isn't. From day 90 onward, we shift from build to scale — hardening MVPs, expanding pilots, and using the enablement layer to ensure the team owns what's been shipped.
The Day-90 Proof-of-Value Checkpoint
At day 90, P1 and the engagement team review working POCs against the success criteria set in week one. If the evidence supports it, the engagement continues through the full six months. If not, P1 has a clean opt-out with no further commitment. The risk is on us, not on you.
Three principles
that shape every week.
P1 doesn't need another vendor relationship. The team needs an AI partner who shows up, ships working things, and helps the internal organization get stronger as the engagement progresses. These three principles shape how we work, week to week.
Ship something working — every two weeks
No 12-week build cycles. Every two weeks, P1 sees something running. The cadence keeps momentum high and gives the team chances to redirect before too much is built in the wrong direction.
Transfer ownership as we go
We embed inside P1's existing rhythms — your AI council, your finance team, your HR leadership. Every POC ships with a P1 owner. The goal isn't to make P1 dependent on us; it's to make the org stronger.
De-risked commercials
Monthly retainer with a built-in opt-out at day 90. We earn the next month by showing value in the prior one. Pricing is transparent. Scope changes get re-priced openly.
Three paths —
same partnership, different intensity.
All three options run for six months with the day-90 opt-out built in. The Enablement Retainer covers the fractional AI support layer alone. The Momentum Retainer bundles enablement with three of the named workstreams — the right middle ground for P1 to ship concrete wins without going all-in. The Acceleration Retainer is the full engagement: enablement plus all five named workstreams and the wildcard.
Ranges reflect natural variation in scope across P1's locations and the wildcard activation. We'll land a fixed monthly number after a one-hour scoping call.
What we'd do this week.
If P1 wants to move forward, the path is short. We'd target a kickoff in the next two weeks and have the first workstream POC underway by week three.
Scoping call — 60 minutes
One call with the engagement leads to confirm option (A, B, or C), set the day-90 success criteria for each workstream, and lock the monthly number.
Kickoff workshop — half day
Sit with the named owners for each workstream. Confirm data access, define the POC scope, and ship a week-one plan with named deliverables.
Week-one ship
Within seven days of kickoff: enablement layer running, office hours scheduled, first workstream POC underway. Momentum from day one.
